Each prospective applicant and his/her parents/guardian must do the following:
- Arrange an interview with the Kindergarten Supervisor.
- Submit a certified copy of the student's birth certificate.
- Submit a State of Florida immunization record and physical form.
- Submit a copy of the student's social security card.
